Position Purpose
This position is responsible for all quality programs and initiatives for the Home Care and Hospice agencies. This position utilizes data analysis to drive process improvement initiatives and promote evidence-based practice. This role will supervise all Home Health Outcomes Specialists and Clinical Documentation staff for both agencies in an effort to ensure a unified approach to quality improvement. Additionally, infection prevention, patient experience, and emergency management will be primary responsibilities of this role. This position requires collaboration with other agency leaders and staff.
Nature and Scope
This position will oversee all chart audits and data collection. Analyzes data collected and makes recommendations for process improvement. Manages annual QAPI plans for both agencies. Manages infection prevention programs. Drives improved outcomes and supervises direct reports to ensure appropriate review and staff education is occurring. Acts as main quality, patient experience, and emergency management representative for regulatory surveys. Assists in driving patient experience initiatives and encourages staff participation. For emergency management, the manager will maintain internal emergency operations plan, implement drills, and ensure compliance with regulatory requirements.

Minimum Qualifications

Name | Description |

Education: | Ability to read, write, speak, and understand English sufficiently to perform job duties safely and effectively. A bachelor’s degree is required, nursing or healthcare related field strongly preferred. Master’s degree highly preferred. |
Experience: | Minimum five (5) years of quality/emergency management/regulatory experience required. Two (2) years of leadership experience strongly preferred. |
License(s): | None |
Certification(s): | Certification as a Certified Professional in Healthcare Quality preferred. |
Computer / Typing: | Must be proficient with Microsoft Office Suite, including Outlook, PowerPoint, Excel, Teams, and Word and have the ability to use the computer to complete online learning requirements for job-specific competencies, access online forms and policies, complete online benefits enrollment, etc. |
