This free workshop will help you learn about and complete a Nevada Advance Health Care Directive (also called a Durable Power of Attorney for Healthcare) and understand what a Provider Order for Life-Sustaining Treatment (POLST) form is. All necessary forms will be provided.

What to expect:

  • Learn the purpose and importance of Advance Health Care Directives.
  • Receive a step-by-step review of the Advance Health Care Directive to help you make informed health care decisions.
  • Learn about POLST forms and who may benefit from having one.
  • Ask questions and receive guidance from Renown staff.
  • Renown staff can serve as witnesses and upload completed Advance Health Care Directives to your Renown medical record and the Nevada Lock Box Registry.

PLEASE NOTE: Notary services will not be available, and copies of completed documents will not be provided. A list of notary services will be provided.

Please bring a photo ID, the names, addresses, and phone numbers of the individuals you would like to designate as your healthcare decision-makers.
