Comparing Temporally-Modulated Pulsed Radiation Therapy (TMPRT) Versus Standard Radiation Therapy With Temozolomide for Adults With Newly Diagnosed MGMT-Unmethylated Glioblastoma

The purpose of this study is to test if pulsed radiation therapy (TMPRT) can reduce decline in thinking and memory skills after treatment. This study will help the study doctors find out if this different approach is better than the usual approach. To decide if it is better, the study doctors will be looking to see if the pulsed radiation therapy reduces the decline in memory and thinking compared to the standard radiation therapy.

• Adults 18+ years of age
• Confirmed diagnosis of IDH-wildtype glioblastoma with unmethylated MGMT promoter
• No known IDH mutation
• No known leptomeningeal disease or metastatic disease outside the brain
• No prior cranial radiation therapy that would result in overlap of radiation therapy fields

• Random assignment to receive either temporally-modulated pulsed radiation therapy (TMPRT) or standard radiation therapy given daily, 5 days a week, for 3 or 6 weeks
• Temozolomide, a chemotherapy drug, taken along with radiation
• Imaging scans with magnetic resonance imaging (MRI) and computed tomography (CT)
• Blood sample collection
• Completing tests of memory and thinking, and questionnaires about your quality of life and day-to-day functioning
