Anthem Blue Cross and Blue Shield Medicaid members will now have access to providers in the value-based Western Clinical Alliance, powered by Renown Health. RENO, Nev. Hansen, the Hometown Health team introduced a Transitional Care Navigator program resulting in reduced hospital readmission rates, added Renown-branded insurance products on Nevada Health Link, Nevada’s Health Insurance Exchange and helped to grow Senior Care Plus, Hometown Health’s Medicare Advantage Plan membership during the 2022 Annual Election Period. Hansen and the Hometown Health team implemented a number of Total Cost of Care initiatives in partnership with Renown Medical Group to make premiums and health care costs more affordable, including health outreach efforts and medical screenings to improve health care for members over age 65.
